Applicants must be Arizona residents diagnosed with end-stage renal disease (ESRD) or major organ transplant recipients for whom alternative sources of funding are not available.
Awards will be made to applicants enrolled in Arizona community colleges, universities, and vocational schools.
NKF AZ does not assist with car insurance payments, emissions testing, registration, or reimbursements for car repairs that are completed prior to submitting a request.
It is NKF AZ’s policy not to assist patients with payments they cannot maintain on their own. For car payment assistance, the patient must provide a plan for how they will avoid the need for future car payment assistance.

Patients are eligible when their regular mode of transportation is not available; there are no family, friends or neighbors who can transport them; and they cannot afford to pay for other means of transportation.
Household income at or below 400% of Federal Poverty Level with liquid assets not exceeding $2,000 (See FPL Chart)

Patients must first qualify for the taxi voucher program through the City of Glendale; after qualifying for that program, the patient must demonstrate financial need, per the Financial Statement.

Patients are eligible when there are no family, friends or neighbors who can transport them; and they cannot afford to pay for tickets, vouchers, or fares.
Household income at or below 400% of Federal Poverty Level with liquid assets not exceeding $2,000 (See FPL Chart)
The maximum household income cannot exceed 200% FPL (See FPL Chart). Income for ALL individuals living at the address must be reported on the Financial Statement.
NKF AZ will pay up to two months in arrears for a maximum of $800.
NKF AZ does not assist with late fees, reconnect fees, or other penalties. Patients must be prepared to pay any remaining balance not covered by NKF AZ in order to prevent the utility shutoff.
This assistance is limited to a one-time assistance for either rent (including the rent eviction prevention assistance) OR utilities.
Patients who have previously received rent or utility assistance with NKF AZ prior to January 2025 will not be eligible to receive this assistance.

NKF AZ will pay up to two months of rent, either both months in arrears, or one month in arrears and one month forward for a maximum of $3,200.
