Food Assistance

This program provides assistance in the form of grocery store gift cards for new ESRD patients experiencing financial hardships as they adjust to the dialysis routine and new diet restrictions. These patients are also eligible to receive a hard copy of the NKF AZ cookbook, available in English or Spanish.

Assistance is limited to a one-time assistance of $280 per patient.

Applications are processed on the second Tuesday of each month.

If approved, the card will be sent to the patient's clinic; expect up to a week for it to arrive by postal mail. If the application is declined or requires additional information, the applying social worker or dietitian will be notified by email. 

Eligibility (in addition to the General Eligibility Requirements on main page):

  • Assistance must be requested by a renal dietitian during the patient’s first three months of dialysis as an ESRD patient, with an explanation of the patient’s need for help and their difficulty adjusting to dialysis and/or the renal diet.

  • The Renal Registry form must be completed and signed by the patient.

  • Household income at or below 400% of Federal Poverty Level with liquid assets not exceeding $2,000 (See FPL Chart)
