This program provides assistance in the form of grocery store gift cards for new patients experiencing financial hardships as they adjust to the dialysis routine and new diet restrictions. These patients are also eligible to receive a hard copy of the NKF AZ cookbook. Assistance is limited to a one-time assistance of $280 per patient.
Eligibility (In addition to the General Eligibility Requirements above):

Assistance must be requested by a renal dietitian during the patient’s first three months of dialysis with an explanation of the patient’s need for help and their difficulty adjusting to dialysis and/or the renal diet.

The Renal Registry form must be completed and signed by the patient.

A copy of the NKF AZ Cookbook: Guide for Renal Nutrition will be issued along with the grocery store gift card. A Spanish translation of the cookbook is available upon request.
