Transportation - Fuel Assistance

The primary purpose of the fuel assistance program is to aid patients living in rural parts of Arizona, who must travel long distances to dialysis. Fuel assistance is also available to patients who use a privately owned vehicle to get to and from dialysis and other medical appointments and are struggling to afford fuel. Gas Card disbursements cover a three-month period.

Effective January 1, 2024 there are two tiers of fuel card assistance. 

  • Patients who drive 15 miles or less round trip will receive the "Local" fuel card, disbursed in increments of $120 to cover a 3-month period. 
  • Patients who drive more than 15 miles per round trip will receive the "General" fuel card assistance, disbursed in increments of $240 to cover a 3-month period. 
  • Patients who drive 100 miles or more per round trip will recieve the "Remote" fuel card assistance, disbursed in increments of $480 to cover a 3-month period. This level of assistance may include a monthly spending cap of $160/month during the covered period.

NKF AZ will review applications from time to time to confirm the distance reported in the application. If the travel distance in the application is misreported, the patient may be disqualified from future assistance, so please be as accurate as possible in your applications.
