Assistance with emergency cab rides is limited to the patient’s immediate needs. Each one-way trip or round trip requires prior authorization from NKF AZ. Social workers requesting this assistance for their patients will be responsible for coordinating the ride, once authorization is received from NKF AZ. NKF AZ will not reimburse for cab rides that have not been authorized.
Currently available only in the Phoenix area.
Eligibility (In addition to the General Eligibility Requirements above):

Patients are eligible when their regular mode of transportation is not available; there are no family, friends or neighbors who can transport them; and they cannot afford to pay for other means of transportation.
