This assistance is available to patients who use their own car for transportation to dialysis and other medical appointments. Assistance is available with car payments and repairs. Car payment or repair assistance is limited to once within a 12-month period, a maximum approval of $500.
NKF AZ does not assist with car insurance payments, emissions testing, registration, or reimbursements for car repairs that are completed prior to submitting a request.
It is NKF AZ’s policy not to assist patients with payments they cannot maintain on their own. For car payment assistance, the patient must provide a plan for how they will avoid the need for future car payment assistance.

Car Payment/Repair Assistance Consent Form giving NKF AZ permission to contact the loan or repair company must be signed by the patient.

When ownership of the vehicle involves someone other than the patient, an explanation is required of responsibility for payment.
