This assistance is available to patients who use their own car for transportation to dialysis and other medical appointments. Assistance is available with car payments and repairs.
Car payment or repair assistance is limited to a one-time assistance for either car payment or repair, a maximum approval of $500. Patients who have recieved rent or utility assistance must wait a minimum of 12 months before applying for car payment/repair assistance.
NKF AZ does not assist with car insurance payments, emissions testing, registration, or reimbursements for car repairs that are completed prior to submitting a request.
It is NKF AZ’s policy not to assist patients with payments they cannot maintain on their own. For car payment assistance, the patient must provide a plan for how they will avoid the need for future car payment assistance.
Eligibility (in addition to the General Eligibility requirements):

Household income at or below 400% of Federal Poverty Level with liquid assets not exceeding $2,000 (See FPL Chart)

Car Payment/Repair Assistance Consent Form giving NKF AZ permission to contact the loan or repair company must be signed by the patient.

When ownership of the vehicle involves someone other than the patient, an explanation of the patient's responsibility for payment is required.

Financial Statement (if not already completed within the last six months for other assistance requests)
- If this is the first time the patient has requested assistance from NKF AZ, they must complete a New Applicant Registration form. Please attach this form to the application.
