Patient Assistance Programs

National Kidney Foundation of Arizona (NKF AZ) patient assistance programs are intended to prevent patient hardships from undermining medical stability and quality of life. We prefer to split financial assistance with other agencies, organizations, health care providers, patients, and their families. Assistance from NKF AZ is the ‘last resort’ when assistance from other resources is not available.

We do not make payments payable to the patient. We do not reimburse patients for payments already made. We do not assist with bills from the past, such as car repairs that have been completed and for which the patient has made arrangements to pay, or credit card charges, nor do we assist to repay loans from family members. We do not help with insurance premiums, doctor bills, hospital bills, dialysis costs or transplantation costs.

Certain limits of programming are noted below followed by the parameters of eligibility determination, forms to be submitted, and renewal requirements.

GENERAL ELIGIBILITY REQUIREMENTS – ALL PROGRAMS

For all NKF AZ patient assistance programs, individuals must meet the following criteria:

  • Have a diagnosis of end-stage renal disease (eGFR <15) or be a kidney transplant recipient.

    • Living organ donors can also apply for assistance up to a maximum of $1,000

  • Participate in an ongoing treatment program (dialysis / transplantation)

  • Be an Arizona resident (proof of residency required)

  • Be a citizen of the United States, a legal permanent resident, or qualify under Federal Emergency Services (FES)

  • Meet financial eligibility requirements - household income that is at or below 400% of Federal Poverty Level guidelines.

  • Have completed a Financial Statement within the past six months that provides evidence of financial hardship based on total household income and monthly expenses

  • Be referred by a renal social worker, dietitian, or prescribing nephrologist office
