Pre-Listing Transplant Medications

Patients who are listed for transplant and struggle to afford the expected cost of their transplant medications can receive assistance with NKF AZ. At the time of transplant, patients will receive a maximum of three months of medications at a time.

Eligibility (In addition to the General Eligibility Requirements above):

  • Assistance is available for the 20% copay for immunosuppressants that is not covered by Medicare Part B.

  • Assistance is available with the cost of high medication copayments.

  • Patient must be approved to be a transplant candidate

  • Must have insurance coverage

  • Submit copies of insurance cards, ID, list of medications (subject to change at the time of transplant).

  • Medication coverage is only offered for those medications listed on the NKF AZ formulary. The program is mandatory generic when a generic exists.

  • The patient is charged a $6.00 copay per prescription for generic medications and a $12.00 co-pay for brand name medications payable to the pharmacy at the time the prescription is filled. Prescriptions are filled for one month at a time.

  • For “Dispense as written” prescriptions that specify a brand name drug when a generic is available, NKF AZ will cover the cost of the generic. The patient will be responsible for the difference in cost between the brand name and the generic.

  • NKF AZ will review active applications at the end of each fiscal year and reach out to request transplant status of the patient and, if still on the waitlist, request an updated financial form.
