The focus of our transportation assistance is to enable patients to get to and from dialysis or pre-/post-transplant medical appointments. Assistance is limited to 26 rides per month for regularly scheduled transportation.
In the City of Phoenix, Dial-a-Ride and Bus passes are available.
Qualifying patients are approved for up to 6 months of assistance at a time.
Eligibility (in addition to the General Eligibility Requirements on main page):

Patients are eligible when there are no family, friends or neighbors who can transport them; and they cannot afford to pay for tickets, vouchers, or fares.

Household income at or below 400% of Federal Poverty Level with liquid assets not exceeding $2,000 (See FPL Chart)

Patients whose insurance provides non-emergency transportation are not eligible.
