The focus of our transportation assistance is to enable patients to get to and from dialysis or pre-/post-transplant medical appointments. Assistance is limited to 26 rides per month (not to exceed $100 per patient) for regularly scheduled transportation.
Eligibility (In addition to the General Eligibility Requirements above):

Patients are eligible when their regular mode of transportation is not available; there are no family, friends or neighbors who can transport them; and they cannot afford to pay for tickets, vouchers, or fares.
